MediShield Life premiums, by age — the official table

MediShield Life is the government hospital insurance every Singapore Citizen and PR carries for life — and its premium is paid entirely from MediSave, never cash. This is the full schedule effective 1 April 2025 (Table B: before subsidies, inclusive of 9% GST), verbatim from CPF’s official premium table — the same dataset our hospital-plan comparisons compute with.

How this sits under an Integrated Shield Plan

Layer 1 — MediShield Life (this table)Everyone’s base layer, sized for B2/C-class public wards. Fully MediSave-paid, identical under every insurer — so it never differentiates plan choices.
Layer 2 — the insurer’s additional premiumWhat an Integrated Shield Plan adds for A-class/private cover. MediSave-payable up to the Additional Withdrawal Limit; the rest is cash. This is where plans genuinely differ.
Layer 3 — the riderCuts your co-payment; always paid in cash. The 2026 rider reform (S$6,000 co-payment cap on new riders) reshaped this layer — see the 2026 rider changes.

Quick answers

How much is MediShield Life per year?Under the schedule effective 1 April 2025 (before subsidies, inclusive of 9% GST), annual premiums range from S$200 at Age Next Birthday 1–20 to S$2,826 above age 90. For example: S$503 at ANB 31–40, S$637 at 41–50, S$903 at 51–60 and S$1,131 at 61–65. Premiums step up when you enter the next age band.
Can MediShield Life premiums be paid with MediSave?Yes — fully. MediShield Life premiums are payable entirely from MediSave with no cash component, unlike Integrated Shield Plan additional premiums (MediSave-payable only up to the Additional Withdrawal Limit) and riders (always cash).
Do I still pay MediShield Life if I have an Integrated Shield Plan?Yes. An Integrated Shield Plan is built ON TOP of MediShield Life — your total premium is the MediShield Life premium (this table, MediSave) plus the insurer's additional private insurance premium (partly MediSave, partly cash) plus any rider (cash). The MediShield Life layer is identical whichever insurer or plan you choose, so plan comparisons should focus on the additional premium and rider.
Are these the subsidised premium amounts? No — this is Table B, premiums BEFORE subsidies. Lower- to middle-income Singapore residents can receive means-tested premium subsidies, and Pioneer and Merdeka Generation seniors receive additional support, so the net premium many people actually pay is lower. Check your personalised figure on your CPF Healthcare dashboard.
Do foreigners pay MediShield Life premiums? No — MediShield Life covers Singapore Citizens and Permanent Residents only. Foreigners in Singapore rely on private medical insurance (for example an insurer's foreigner-tier plan) without the MediShield Life base layer or MediSave premium support.
