How much does an Integrated Shield rider cost?It depends heavily on plan tier and age. At Age Next Birthday 45, comprehensive riders on private-hospital plans run roughly S$540–1,507 a year (Raffles Choice ~S$540, Income Optima S$1,196, PRUExtra Premier ~S$1,226, GE TotalCare 2 P ~S$1,507), while value riders and Class A/B1-tier riders cost far less. By ANB 75 the same private-tier riders reach roughly S$2,750–8,600 a year. The full tables above show every rider at ten milestone ages.
Can I pay my Integrated Shield rider with MediSave?No. Riders sit outside the MediShield Life integration and are always paid in cash — which is why Citizens, PRs and foreigners pay the same rider premium, and why the cash portion of your total hospital-cover bill is dominated by the rider at older ages.
Why did rider premiums change in 2026?MOH's rider reform took effect for new riders from 1 April 2026: new riders can no longer cover the plan deductible and carry a minimum S$6,000 annual co-payment cap — and priced roughly 30% lower as a result. The tables above are the post-reform 2026 premium tables (effective 1 Apr–1 May 2026 depending on insurer). Riders bought before 27 Nov 2025 are grandfathered on old terms — including Income's legacy Plus and Assist riders, whose tables are shown separately.
What are Income's Plus and Assist riders?They're the legacy generation of Enhanced IncomeShield riders, before today's Optima Care and Essential Care — closed to new sales but still held by a large in-force book, and still officially re-priced (Plus from 1 Oct 2025, Assist from 1 Apr 2026). Grandfathered holders keep deductible coverage with a S$3,000 annual co-payment cap. Their full premium tables by plan tier are on this page.
